In-House Providers
As a SimplyElite client a physician and advanced practitioner will be assigned to your facility to provide care to your residents a minimum of 3 days per week.
Multidisciplinary Rounding
Your assigned primary care team and medical specialists will perform multidisciplinary team rounds once per week to ensure all patients receive coordinated, high-quality care.
Educational Seminars
As a SimplyElite partner IntegrityNP will host quarterly education seminars for nursing home staff designed specifically for your facility's needs.
Transitional Care Management
We continue to follow patients up to 30 days post-discharge to help transition their care back to their community provider.
