Eliminating the Gaps in Care
Coordinating the transitions of care in the post-acute care setting can be cumbersome and time consuming. Our care model efficiently tracks patients every step of the way from hospital discharge to in-home medical care.
Skilled Nursing
We embrace an In-House model with our partner Skilled Nursing and Assisted Living Facilities. Each facility is assigned a dedicated primary care provider who rounds on patients on a weekly basis. This model fosters more comprehensive and adaptive medical care for our chronically ill patients.
Transitional Care
To ensure our patients maintain their quality of life and independence at home we help coordinate any necessary home health services. From wound care management to assistance with daily activities of living, IntegrityNP ensures home health services are coordinated upon discharge.
