In-House Provider

As a SimplyElite client a physician and advanced practitioner will be assigned to your facility to provide scheduled care to your residents based on their healthcare needs.
Case Conferences

We schedule monthly case conferences between our providers and facility staff to ensure the highest quality of care for our patients.
Specialty Care Coordination
As a multispecialty practice we are able to provide most services in-house. We also coordinate outside specialty visits when necessary.
Active POA Involvement
IntegrityNP actively engages with our patients' families to ensure transparency and shared decision making.
Chronic Care Management
Our comprehensive chronic care management program targets patients at risk for hospitalization and provides them with additional preventative services.
Dedicated Facility Liaison
As a SimplyElite partner your facility will be assigned a dedicated Facility Liaison who is responsible for ensuring your patients receive excellent care in a timely manner.
