We are a Nurse Practitioner owned multi-specialty mobile medical practice of nurse practitioners and physician assistants all of whom are supervised by partner physicians.
What We Provide
A consolidated, multi-disciplinary approach to the care of patients within the post-acute care setting. With the geriatric population suffering from a multitude of disease states, uncoordinated care often leads to unnecessary nosocomial injury and hospitalizations. Our coordinated approach leads to decreased polypharmacy and better health outcomes for our patients.
